1. Fill out the form according to the highlighted example below.

2. Collect 1-7 signatures per page. (You do not need all 7)

3. Fill out the bottom of the form as the certifier that you are collecting the signatures.

4. Get the forms back to the campaign team.

IMPORTANT:

• Forms MUST be received by May 15th to give us time to process them with the election commission. 

• All electors must reside in the 96th Assembly District. Check the map HERE to make sure you are in District as the maps have changed.
